| Question Number 1: In accordance with the Freedom of Information Act 2000 I would like to make a request for information. If your hospital has grouped with other hospitals and now shares a Chief Executive and/or other lead/executive roles across more than one hospital please can I request the organisational structure of this shared team, this should include: - structure of team, and who each person feeds down to in each hospital - job title and salary for each team member if outside the agenda for change banding structure - budget and total spend on staff and capital for this structure for each of the past 5 financial years (or however long the structure has been in place) - how much money this structure was expected to and has actually saved your trust(or group) for each of the past 5 financial years(or however long the structure has been in place) | |

| Question Number 2: Please can I request the organisational structure of you Transformation team(or equivalent), this should include - structure of the team - job title, band level, and WTE for each staff member - list of data areas and systems that each staff member is the leads for/is the service first point of contact. - budget and Total spend on staff and capital for each of the past 5 financial years - the amount the team has saved the Trust money for each of the past 5 financial years, this should only include projects where the lead person was a member of the transformation team Please include any staff member that is currently seconded away but is expected to return to the team. It is not neccesary to include the name of the staff members but please could you provide the contact details for the head of department. | |
